GP Referrals
GP referrals are welcomed, including referrals under a Mental Health Treatment Plan (MHTP), GP Chronic Condition Management Plan (GPCCCMP), DVA, NDIS and Workcover or Comcare.
Referrals and supporting documentation can be sent via one of the following:

Standard Communication with Referring GPs
Acknowledgement of referral and confirmation of engagement with services
Progress review following the 6th session
Progress review following the 10th session
Cessation of engagement letter
Additional communication, recommendations, or referrals where clinically appropriate and with the client’s consent, to support continuity of care and treatment planning.
